The drum is driven by a power take-off from the engine of the truck.

Inside the drum are a series of spiral vanes. These keep the cement mixed and prevent the sand, cement and aggregate from being separated out while being transported. On site, the drum revolves in the opposite direction and the spiral vanes act as an Archimede's Screw to drive the mixture out of the drum and into the delivery chutes at the rear.

Because the mixing ribs inside the drum are spiraled so the anti-clockwise pushes the concrete inward.  When reversed, it pushes the concrete up and out of the drum.
